The FORM screen (but not the WIZARD) screen can be customized.

Existing fields can be changed.

To change the screen:
Open a command box (DOS) box
Navigate to the directory where HELPDESK.EXE is installed
At the command prompt type:
helpdesk.exe /admin
Login as ADM (aministrator)
Open the FORM screen
Place the cursor on or near the field to change and press Ctrl+S
Change the field properties.
